Why is a full history so important?

Before your assessment, it is crucial that  comprehensive screening and background questionnaires are completed in advance.  This allows your assessor time to review the information, ask additional questions and plan for any additional tests needed, or make a referral to a medical professional if more appropriate.

This information is also vital for providing evidence of long-term and substantial issues (which is needed to comply with the Equality Act 2010) and make a formal diagnosis.

 

What Information do I need to provide?

​

You will be asked about:

  • Your early childhood development

  • Family 

  • Basic medical history

  • Education and work history

  • Language

​

You will then be asked to  give details about the areas  where you have issues, including:

  • Literacy issues (reading, writing, spelling)

  • Coordination, organisation, attention

  • Orientation, memory and time management

  • Vision
